Rural Outreach Navigator - OHH ESG

Position Summary

The OHH ESG Program Case Manager is responsible for providing outreach, housing-focused case management, shelter coordination, rapid rehousing support, and housing retention services to individuals and families experiencing homelessness or at risk of homelessness.

The Case Manager serves as a primary point of contact for participants and supports them through outreach, Coordinated Entry, shelter placement, housing search, move-in, and housing stabilization. This position requires regular field-based work throughout Fresno and Madera Counties.

Essential Duties and Responsibilities

The essential functions include, but are not limited to the following:

● Conduct outreach and engage individuals and families experiencing homelessness or housing instability

● Complete screenings, assessments, eligibility reviews, and required program documentation

● Assist participants with Coordinated Entry enrollment, referrals, and housing opportunities

● Coordinate emergency shelter placements, transportation, and temporary housing resources as needed

● Develop individualized housing plans that identify participant goals, strengths, and barriers to housing

● Assist participants with housing searches, applications, document collection, unit identification, and move-in planning

● Coordinate approved housing-related financial assistance and communicate with landlords or property managers when appropriate

● Provide ongoing housing stabilization services, including follow-up contacts, home visits, lease compliance support, and connection to community resources

● Identify and address barriers that may place a participant’s housing at risk

● Maintain working relationships with landlords, shelter providers, community agencies, healthcare providers, behavioral health providers, and other housing partners

● Maintain accurate and timely case notes, participant records, HMIS entries, and required reports

● Participate in outreach events, community meetings, and program meetings as assigned

Minimum Qualifications (Knowledge, Skills, and Abilities)

● Minimum of 2 years of experience working with individuals experiencing homelessness, housing instability, behavioral health challenges, or similar populations preferred

● Experience in outreach, case management, housing navigation, rapid rehousing, shelter coordination, or community-based services preferred

● Knowledge of housing resources, homeless response systems, Coordinated Entry, HMIS, and community services preferred

● Strong communication, organization, problem-solving, and documentation skills

● Ability to engage individuals from diverse backgrounds while maintaining professional boundaries and confidentiality

● Ability to work independently, manage multiple priorities, and complete timely follow-up

● Knowledge of trauma-informed care and Housing First principles preferred

● Valid California Driver’s License, proof of insurance, and clean driving record required

● Ability to travel throughout Fresno and Madera Counties and work in office, shelter, housing, and field-based settings
