CalAIM CoC System Coordinator

Position Summary

The CalAIM System Coordinator understands how CalAIM services support the needs of individuals experiencing homelessness and the role of CalAIM within the local response system. The coordinator has a dual purpose of identifying opportunities for collaboration and integration within FMCoC and in the community. The coordinator extends CalAIM knowledge to FMCoC membership and supports operational integration of CalAIM within the local response system operations.  This position serves as a point of contact for CalAIM providers outside of the Fresno Madera Continuum of Care. 

Essential Duties and Responsibilities

The essential functions include, but are not limited to the following:

  • Help FMCoC membership and the community understand how CalAIM can assist people experiencing homelessness
  • Assist with identifying gaps in the local response system of care and opportunities for building around areas of need
  • Partner to ensure HMIS becomes an effective tool for accurately measuring CalAIM referral and enrollment outcomes
  • Assist Managed Care Plans and the FMCoC with HMIS bi-directional data sharing.
  • Meet with local providers and assist with program and budget development to braid CalAIM funding into operations
  • Support providers with designing effective workflows for integration of CalAIM to increase CalAIM referrals  
  • Facilitate the participation of ECM and Community Supports Housing Transition Navigation Services providers in the FMCoC’s Coordinated Entry System (CES).
  • Meet with CalAIM providers to understand their program services, their implementation barriers, and support their role within FMCoC and the local response system, including how to navigate the CoC, understanding benefits of participation, and successful onboarding into the CoC system
  • Facilitate connections between CoC providers and CalAIM providers for individual clients, including support through case conferencing meetings to address complex case needs

Minimum Qualifications (Knowledge, Skills, and Abilities)

  • 2 years experience in housing related social services
  • Management experience preferred
  • Cleaning driving record
