Position Summary

The Housing Navigator supports participants in securing and maintaining permanent housing by

assisting with housing searches, placement, and coordination of supportive services. This role is

responsible for helping clients gather required documentation (income verification, identification, etc.),

completing housing applications, engaging landlords, and ensuring timely and accurate data entry. The

Housing Navigator plays a critical role in advocating for client choice and promoting long-term housing

stability.

Essential Duties and Responsibilities

The essential functions include, but are not limited to the following:

● Utilize best-practice approaches such as Housing First, Harm Reduction, Progressive

Engagement, Motivational Interviewing, and Trauma-Informed Care.

● Conduct tenant screenings and housing assessments to identify needs, preferences, and potential

barriers.

● Develop individualized housing support plans with measurable goals.

● Assist clients with completing rental applications, gathering required documents (ID, Social

Security card, income verification, rental history), and navigating housing requirements.

● Support clients with benefits advocacy, such as SSI applications or income documentation.

● Assist in preparing reasonable accommodation requests when appropriate.

● Identify and coordinate rental support resources (Section 8, local housing programs, rapid

rehousing, etc.).

● Assist in securing funds for deposits, moving costs, adaptive aids, and one-time household setup

expenses.

● Maintain complete, accurate client files and ensure all documentation is completed within

required timeframes.

● Provide landlord education on Community Supports, program services, and member support

expectations.

● Build and maintain positive relationships with landlords to increase housing opportunities.

Minimum Qualifications (Knowledge, Skills, and Abilities)

● Minimum of 2 years of experience in case management, care coordination, or a related social

services role

● Knowledge of the Fresno Madera Continuum of Care and Housing Services

● Prior experience working with individuals experiencing homelessness, serious mental illness,

substance use disorder, or complex physical health conditions preferred

● Familiarity with Medi-Cal, CalAIM, or managed care systems is a plus

● Excellent interpersonal, organizational, and written communication skills

● Proficient in documentation and case noting in electronic systems

● Clean driving record
